What Services Are Provided?
Once admitted to Hospice each patient will be surrounded by a team of experts to provide the best care possible. Our team of physicians, nurse practitioners, registered nurses, social workers, chaplains and volunteers will provide:
- Expert Medical Care – Our nurse will help to manage any symptom your loved one may experience, such as constipation, uncontrolled pain , anxiety or shortness of air. Our Nurse will work closely with your Physician to provide the best care possible. Our Nurse will make at least one visit per week and are available 24 hours a day.
- Emotional Support – Each patient will have a social worker on their team to support the patient and their family. Our experienced Social Workers can help with confusing paperwork, such as insurance forms or other legal documents. Our social workers will also help with funeral arrangements and provide any other emotional support needed.
- Spiritual support – The hospice chaplains are available to give spiritual support during the final stages of life. Our hospice Chaplain helps our patients have meaningful discussion about hope, forgiveness and peace.
- Help with everyday activities – Our Hospice Care Aides help provide personal care to our patients including bathing and feeding. Our hospice care aides also help educate the families and care givers on proper care. The aide visits the patient twice a week.
- Companionship – Our hospice trained volunteers help provide support to the patient and their families. Our volunteers can provide a much needed break for caregivers and offer companionship to patient.
- Medication Management – Our team continually works with your loved ones to adjust and monitor prescriptions. All medications related to the individuals terminal diagnosis will be ordered and refilled by St. Anthony’s Hospice. They can also be delivered directly to the patients’ doorstep at no cost to the patient.
- Durable Medical Equipment – Our patient may have a sudden need for medical equipment, such as a hospital bed, oxygen or a wheelchair. For patients of hospice care, we will have the necessary equipment delivered to the patients home at no cost to the patient.
- Medical Supplies – As our patients decline, many families and caregivers need supplies to properly care for their loved one. Those supplies, such as boost, gloves and briefs are provided to hospice patients to improve their quality of care.
