How to get started with Hospice or Palliative Care.
Anyone, not just a medical professional, can make a referral to St. Anthony’s Hospice or Palliative Care. Once you or a loved one have been diagnosed with a serious illness, getting started with St. Anthony’s Hospice or Palliative Care program requires either a phone call to (270) 826-2326 or filling out the below form.

We take referrals 24 hours a day, 7 days a week and allow anyone to make a referral. To begin the referral process, please call (270) 826-2326 or fill out the below form. We will help you through every step of the way. Once we get some basic information, our referral specialist will schedule an appointment for one of our admission nurses to come talk to the patient and family about how we can help. After a few papers are signed, St. Anthony’s can begin services.
