Palliative Eligibility

Anyone with a chronic, life-limiting, or terminal illness qualifies for Palliative Care! Please call St. Anthony’s to learn more about Palliative Care or to sign up.

General Eligibility Requirements:

• Need to establish or clarify goals of care
• Need help with Living Will, Power of Attorney, or Code Status
• Multiple hospitalizations due to same diagnosis
• Require assistance with 3+ Activities of Daily Living (ADLs)
• Lives alone and/or high caregiver burden
• Progressive decline over the past 6 months
• Needs symptom management
• Needs education on disease process
• Need assistance with community resources
• Specific decline of condition- AIDS, cancer, COPD, heart failure, stroke, Alzheimer’s, liver disease/cirrhosis, neurological conditions (Parkinson’s, MS, ALS, Huntington’s, etc.), renal disease, etc.
