Position Summary and Purpose:
The Hospice chaplain is responsible for creating and nurturing a spiritual presence throughout the organization. This person will also provide on-going, direct ministry to patients, family, the Hospice team, in addition to developing positive relationships with the community clergy.
Qualifications:
- Undergraduate degree in applicable subject; Master’s degree in theology, divinity, or pastoral studies; Internship or Clinical Pastoral Education (separately or concurrently with master’s degree)
- Experience in the ministry or in a chaplaincy spiritual counseling position.
- Ability to provide spiritual care without value judgment on personal spiritual beliefs, and belief in the hospice concept of terminal care.
- Must arrange travel and be able to access client homes independently.
- Must have ability to work independently of direct on-site supervision.
- Must carry $100,000/$300,000 automobile liability insurance.
Personal:
- Must carry a minimum of $100,000 automobile insurance.
- Must be able to pass Criminal Records Background Check.
- Must be able to pass the Driving Records Check upon hire and annually.
Responsibilities and Duties:
- Participates as an active member of the Interdisciplinary Team for the assigned territory.
- Completes spiritual assessment of each patient/family, and follows up as needed.
- Provides support and spiritual care to patients and family in keeping with their beliefs.
- Conducts or makes arrangements for funeral or memorial services as needed.
- Advises team of problems concerning patient or family.
- Acts as a liaison with the religious community, and advises patient’s pastor of patient’s involvement in the Hospice Program.

Reports To: Director of Nursing
Position Summary: The RN is responsible for planning, coordinating, providing and documenting care for terminally ill patients from day of admission through day of discharge. Utilizing physicians’ orders and professional skills, the Hospice RN develops and implements a hospice Plan of Care that meets each patient’s specific needs and is in compliance with federal and state regulations as well as St. Anthony’s standards and policies.
Essential Job Functions:
- Provides physical and psychosocial assessments on assigned hospice patients.
- Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary.
- Provides hospice level nursing care in conjunction with Medicare Conditions of Participation and hospice regulations and standards.
- Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
- Provides examinations, care and treatments including but not limited to measuring vitals, catheter and colostomy care, care of wounds, pressure sores and stasis ulcers, respiratory care, enteral feedings, care of central venous access devices, use of PCA-CADD pumps, and post mortem care.
- Administers medications based on ages of the patient and the patient’s clinical position.
- Orders needed medications, supplies, and equipment and observes and documents its’ use and effectiveness towards the goal of the patient’s comfort and safety.
- Assesses/reassess pain, utilizes pain management techniques and carries out the pain/symptom management plan to keep the patient as comfortable as possible.
- Instructs the patient/family members in patient safety; explains medications, treatments, and procedures; provides information regarding the disease process including death and dying.
- Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.
- Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, new problems, etc.
- Completes clinical documentation that supports daily tasks and patient’s Plan of Care.
- Communicates with patients, families, physicians and team members to share and elicit information related to patient care and treatment.
- Participates in staff meetings, inservices, special team meetings, etc. as assigned or requested by the LSKCC Director.
- Completes other duties as assigned.
Professional Qualifications:
- Must be a Registered Nurse in the State of Kentucky
- Previous nursing experience is required. Experience preferably in Hospice or in a medical/surgical setting, a hospital, oncology or geriatric facility.
- Familiarly with the use of computers preferred including internet navigation and email use.
- Must demonstrate commitment to Hospice concept of care.
- Must meet all legal and regulatory requirements of both federal and state governments as well as Medicare Hospice Conditions of Participation and JCAHO.
Physical Qualifications:
- Must be able to remain in a stationary position to complete patient documentation.
- Must be able to frequently move throughout the LSKCC to attend to the needs of patients and families.
- Must be able to communicate verbally, via email and in writing with patients, families, nursing staff, physicians and visitors.
- Must be able to lift 50 pounds and push/pull 50 pounds, 8-12 times per day to perform essential job functions of bathing, transferring and providing general patient care.
- Regularly exposed to blood, body fluids, cleaning chemicals and other potentially harmful substances while caring for patients and completing housekeeping duties. Must be able to wear appropriate Personal Protective Equipment.
- Must be able to use medical equipment and devices needed to care for and treat patients.
- Must be able to operate a computer and basic office equipment.
