Anyone can initiate a referral call to St. Anthony’s Hospice. Here’s how it works:
- Call us at 270-826-2326 and provide the name of the family and their phone number
- St. Anthony’s will call the family to gather information and informed consent
- Hospice will also call the patient’s doctor for information about diagnosis and prognosis, orders for medications and treatments, patient history and physical information, and orders for hospice care
- A St. Anthony’s Hospice staff member will make an appointment for a family/patient visit and will create a plan of care based on the nursing assessment
